Healthy Living with T1D

Goal
To improve Type 1 Diabetes (T1D) care in Kenya by strengthening community support systems, enhancing access to safe and nutritious foods, and providing psychosocial support for patients and caregivers.
Project Overview
Healthy Living with T1D is implemented by the Diabetes Awareness Trust (DAT) with support from Panorama T1D. Running from February 2025 to March 2027, the project expands DAT’s Community Diabetes Support Network (CDSN) in Thika Sub-County, Kiambu County.
The initiative combines clinical self-management, nutrition security, and community empowerment, placing patients, caregivers, and health actors at the center of resilient T1D care.
Target Outcomes
- Improved Self-Management & Clinical Outcomes – equipping T1D patients with knowledge, peer support, and monitoring systems for better adherence and quality of life.
- Enhanced Community Capacity & Nutrition Security – ensuring access to safe, fresh, and affordable foods through stronger local food systems.
Key Activities
Outcome 1: Enhanced Community Capacity & Nutrition
- Champion Training Workshops: Bi-annual sessions for patients and parents of children living with T1D.
- T1D Awareness Webinars: Bi-annual 1.5-hour online sessions with medical experts to strengthen knowledge for patients, caregivers, and stakeholders.
- Producer & Vendor Mapping: Identifying 20 agroecological farmers and 5 local vendors to link patients to safe and nutritious foods.
- Hospital Demo-Gardens: Establishing demonstration gardens at Thika Level 5 hospital for practical food production learning.
Outcome 2: Improved Self-Management & Adherence
- Digital Peer-Support Network: Scaling up the CDSN WhatsApp group to reach ~100 T1D patients by enrolling 10–15 new members each quarter.
- Patient Follow-Up & Alerts: Continuous monitoring and personal follow-up on patient progress and challenges, with champions supporting adherence.
The Bigger Picture
This project goes beyond clinical treatment. By integrating nutrition, mental health, and peer-led community support, it strengthens resilience among people living with Type 1 Diabetes. At its core is the belief that managing T1D is not just about access to insulin—it is about building healthier, supportive environments where patients and families can truly thrive.
